Faith, culture and mental health decisions that stand up to scrutiny.

We help NHS trusts, integrated care boards and health charities make and record the decisions where a patient’s faith, culture or family context changes what good care looks like.

Why this matters now.

Faith, culture and family shape how people seek help, what they disclose and how they recover. National frameworks now ask NHS organisations to show, with evidence, how they account for this.

  • Patient and Carer Race Equality Framework (PCREF)

Mandatory for NHS mental health providers under the NHS Standard Contract, with a nominated board-level lead, and part of how CQC assesses services.

    CQC guidance on PCREF
  • Definition of anti-Muslim hostility

Adopted by the Government in March 2026 as a non-statutory definition. The Government has asked NHS England to adopt it and expects trusts, ICBs and arm’s length bodies to do likewise.

  • NHS Leadership and Management Framework

Launched on 6 July 2026, setting national standards for every NHS leader and manager, with organisations asked to begin embedding it during 2026/27.

  • Ombudsman findings

The Parliamentary and Health Service Ombudsman reports persistent failures to consult patients and families about DNACPR decisions. Its discussion of advance care planning includes how a patient wants religious or spiritual beliefs reflected in care.

What this means for you

Each of these asks the same thing: show how you considered the people in front of you, and what you did about it. That is what we help you build and record.

Where faith strengthens your PCREF work.

PCREF is mandatory for NHS mental health trusts and providers, forms part of CQC inspections, and asks trusts to show how they implement, evaluate and achieve its actions and outcomes. Faith and belief sit alongside race in how many patients and carers experience care, so they strengthen each of the three components.

  • Leadership and governanceYour board lead can show how faith and belief were considered in the local plan, and how that reasoning is recorded.
  • DataFaith recorded separately from ethnicity, so patterns in access, experience and outcomes can actually be seen.
  • FeedbackRoutes that reach faith and community leaders and the people who rarely fill in a survey, and a record of what changed as a result.
  • Organisational competenciesStaff confidence in the moments where faith shapes consent, disclosure, family involvement and end-of-life decisions.
What this means for you

Inspectors ask what a provider is doing, how it evaluates it and what changed. A faith review gives your PCREF lead answers to all three, in writing, for the communities most likely to be missed.

Decisions we help with.

The moments where good policy meets a person whose faith or culture changes the right response.

  • Consent and end of lifeResuscitation, withdrawal of treatment and family involvement where faith shapes the conversation.
  • Treatment refusalWhen a declined procedure or medication has a religious or cultural reason behind it.
  • Disclosure and help-seekingHow faith, stigma and family expectations shape what patients say, and when.
  • Safeguarding with a cultural dimensionActing on concern with confidence and without assumptions about a community.
  • Community accessReaching communities for screening, services and research through trusted voices.
  • Leadership capabilityBuilding the cultural capability the new leadership standards depend on.

Questions we are asked

Who helps NHS organisations with mental health across cultures?

CultivIQ delivers Mental Health Across Cultures workshops and diagnostics for NHS trusts, integrated care boards and health charities, and turns what staff, patients and community leaders say into governance a board can evidence.

How do faith and belief fit into PCREF?

PCREF is a race equality framework. For many patients and carers from racialised communities, faith and belief sit alongside race in how they experience care, so including them strengthens the framework’s leadership, organisational competency and feedback requirements.

Can you run a listening exercise with faith and community leaders?

Yes. Community dialogue programmes bring curated groups of faith and community leaders together with your service leads around a specific decision. You receive a written insight summary and a record of what the organisation will change as a result.

Who makes the clinical decisions?

Clinical decisions stay with your clinicians. CultivIQ supports the cultural and faith context around those decisions and how they are made, recorded and reviewed.

Tell us about the decision you are facing.

A first conversation is free. We listen, ask one or two questions, and tell you honestly whether and how we can help.
